Sutter Medical Group (SMG) seeks to hire an outstanding BE/BC Family Medicine Physician seeking a compassionate, highly skilled Family Medicine physician who is board-certified to join our team. This unique opportunity allows you to practice medicine in a personalized, patient-focused environment, providing high-quality, comprehensive care to a select group of patients. As a concierge physician, you will have the time and resources to foster strong, long-term relationships with patients, offering them an elevated healthcare experience.

Key Responsibilities:

  • Provide comprehensive primary care services, including preventive health, diagnosis, treatment, and management of acute and chronic conditions for patients of all ages.
  • Maintain a small, dedicated patient panel, ensuring highly personalized care and accessibility.
  • Conduct thorough patient assessments, physical exams, and medical histories.
  • Develop individualized care plans that focus on long-term health and wellness goals.
  • Engage in proactive, preventative care, including health screenings, immunizations, and lifestyle counseling.
  • Coordinate care with specialists and other healthcare providers as necessary, ensuring continuity and integration of services.
  • Offer same-day or next-day appointments to ensure prompt care and peace of mind for patients.
  • Be available for after-hours consultations, whether via phone, MHO, or telemedicine.
  • Provide education on health maintenance, disease prevention, and optimal wellness strategies.
  • Maintain accurate, up-to-date electronic medical records and comply with all relevant documentation standards.

Services Provided:

  • Extended Appointment Times: Longer visits with patients, which allows for more in-depth consultations and a more thorough understanding of their health needs.
  • 24/7 Access: Provide phone, MHO, or text access by your patients, making it easier to get advice or ask questions outside of normal office hours. Two clinicians per care center with separate practices, but can allow cross coverage for illness and vacation.
  • Same-Day or Next-Day Appointments, during 36 hours per week.
  • Average of 6 patient visits per day – may be in person, virtual or occasionally home or SNF visits.
  • Create individualized health plans tailored to patient’s specific needs, helping to prevent and manage chronic conditions
  • Wellness and Preventive Care: More emphasis on prevention, including regular check-ups, screenings, and proactive care to avoid serious health issues.
  • Coordination of Specialist Care
  • Comprehensive Health Assessments
  • Expected to build and maintain a panel size of approximately 400 patients.

Qualifications

  • Board certification in Family Medicine (required).
  • Active state medical license.
  • Minimum of 3 years of experience in primary care or family medicine.
  • Strong communication and interpersonal skills, with the ability to build strong relationships with patients.
  • Ability to work independently and collaboratively in a team environment.
  • Experience with concierge or direct primary care models is preferred, but not required.
  • A passion for delivering exceptional patient care and improving the overall patient experience.
  • Ability to manage a reduced patient load while maintaining high-quality, individualized care.
